1.Please install Adobe Reader software to open electronic application form. Have the application form mimeographed with A4 copy paper. No page setup should be used while you have it mimeographed.
2.Electronic application form can be mimeographed in color or black and white mimeograph paper. Please ensure that you get a clear copy.
3.It is suggested that both sides of the paper be mimeographed. (If you have it mimeographed page by page, the second page must be enclosed.)
4.Please fill in the application form completely. There is
a signature column of the applicant on each side of the paper. The applicant must sign himself in the signature column.
5.Since you have posted the credit card application form back, we give tacit consent to the fact that you have agreed to the Credit Card Contract of China Minsheng Banking Corp. Ltd on the reverse side of the paper or the second page
6.please deliver this application form and other enclosed testimonials to a counter of our bank's network or a salesman of Minsheng credit card or post them to the credit card center of China Minsheng Banking Corp. Ltd after filling in this form.

Testimonials needed to be enclosed
1.ID testimonials of master card and supplementary card applicant
Residents of the mainland----duplicate copy of ID card or officer's card;
HK, Macao or Taiwan compatriots—duplicate copy of pass of HK and Macao residents to the mainland or Taiwan compatriot card and duplicate copy of residence testimonial for over one year;
Foreigners—duplicate copy of passport and residence permit for over one year;
The supplementary card applicant below the age of 16--- duplicate copy of residence booklet. (It is necessary.)
2.occupation testimonials of master card applicant:
the original occupation testimonial issued by the work unit, duplicate copy of the contract of employment and employee's card and so on .(At least one piece of each testimonial is required.)
3.income testimonials of master card applicant: pay sheet of wages paid by the bank, income testimonial issued by the work unit ,income tax deduction and payment receipt and so on (at least one piece of each testimonial is required).
4.financial power testimonials of master card applicant: title deed for your own house, house mortgage payment receipt, vehicle usage license, vehicle mortgage payment receipt, vehicle purchasing invoice, duplicate copy of fixed deposit sheet in our bank. (These are instrumental to your credit rating in our bank).
5.residence testimonials of master card applicant: payment receipt in latest three months for public service such as water, electricity and coal, for communication such as mobile phone and telephone and for cable TV and property management service. (These are helpful to raise your credit rating.)
6.other testimonials of master card applicant: duplicate copies of professional title certificate and professional qualification certificate. (These will help to raise your credit rating in the bank.)
